*NEW* Implementation Plan Addendum 2025
Counties must complete an IP Addendum for their specifically identified categories by September 3, 2025. The goal of the addendum is for counties to document their continued implementation and sustainability efforts. M-TAC offers ongoing training and technical assistance to support counties.
Only counties that received a separate email from the Medi-Cal Mobile Crisis Training and Technical Assistance Center (M-TAC) team with specific instructions for the IP Addendum Template are required to complete these updates. Please note that counties do not have to complete the entire template—only complete the sections identified by the M-TAC team in the follow-up email. Counties will also receive their previous score for the medium-priority items.
Implementation Plan Addendum Submission Instructions
Please email the completed IP Addendum file to mobilecrisisinfo@cars-rp.org with the subject line: [County/Organization Name] Implementation Plan 2025 Addendum. Submissions must be submitted as a Word document (.docx). Counties will receive a confirmation email from the M-TAC team.
We can accept email attachments totaling up to 150MB. Depending on attachment filesize limits within your organization, you may need to spread attachments across multiple emails.
If the county needs assistance, thought partnership, or any other support to complete the IP, please contact M-TAC by completing the TA request form.

Implementation Plan Downloads
Please find the Implementation Plan (IP) template, as described in Section VIII(c) of BHIN 23-025. Counties must complete an IP and receive written approval from DHCS.
Medi-Cal behavioral health delivery systems in most counties submitted their IPs by October 31, 2023.
Medi-Cal behavioral health delivery systems in the counties of Alpine, Amador, Colusa, Del Norte, Glenn, Inyo, Mariposa, Modoc, Mono, Plumas, Sierra, and Trinity submitted their IPs by April 30, 2024.
Attestation Letter
Counties that meet specific requirements may submit an Attestation Letter confirming that that they meet requirements for implementing the new Medi-Cal benefit. Click here to review the Attestation Letter. Attestation Letters will be reviewed and approved or not approved by DHCS in writing. All counties, regardless of whether they have an approved Attestation Letter, must complete the Implementation Plan.
